Targeted Case Management is a program designed to help clients gain access to their needed medical, clinical, social, and educational services and improve the quality of their lives. Many services are provided by social workers to clients with mental health conditions. Below, you will find some of the most common services and a brief description of them.

Assessment

Areas Of Need: Education, Financial/Basic Needs, Legal, Mental Health/Substance Abuse, Physical Health, Transportation

Targeted Case Managers visit the client and conduct the initial assessment interview to review demographic, medical, personal, and financial documents, as well as identify the client’s needs. After gathering all required information, the Targeted Case Manager lets the client know that they have to complete a service plan based on the identified needs and collected data. Case Managers will also interview the client’s emergency contact. 

Service Plan Discussion

Areas Of Need: Financial/Basic Needs

Case Managers and clients meet to discuss the Initial Service Plan based on the current objectives and needs identified in the Assessment process. The client should agree to cooperate and participate actively in his treatment and services as well as work in collaborative ways towards achieving goals and objectives on the Service Plan. After the client approves and signs, the Case Manager offers a copy of the signed Service Plan to the client, following the agency procedure. As part of the plan, the Case Manager will monitor the client’s treatment compliance, living environment, level of functioning, and medication intake.

Certification

Areas Of Need: Mental Health/Substance Abuse

The Case Manager should certify that the client is not receiving duplicated Case Management services from another provider and is experiencing a long-term impairment that may require more intensive services. The Case Manager should also certify that the client is eligible to receive Targeted Case Management services. After the certification process is completed, an Appendix J Adult Certification or an Appendix I Children Certification should be created and signed.

Housing

Areas Of Need: Housing

Housing services are usually needed when a client is a low-income person who cannot afford the high cost of regular rent expenses. The client needs to find affordable, low-income housing to decrease monthly costs and improve housing conditions. Case Managers assist the client in obtaining information on housing and completing and submitting housing application packages to open housing programs. Before sending the application, the Case Manager must double-check that everything is correct.     

Legal

Areas Of Need: Financial/Basic Needs;

One of the most common needs is to change the client’s legal status. If a client is a low-income person with mental health problems and afraid of losing their benefits, this will cause stress that might affect the client’s mental condition. The Case Manager helps the client prepare the USCIS application package to change their legal status and become a USA citizen. The client needs help to monitor the citizenship process and keep track of when they have their fingerprints and interview appointments. 

Transportation

Areas Of Need: Physical Health

The client needs help activating and scheduling medical door-to-door transportation services to attend appointments with PCP doctors and specialists. The Case Manager coordinates transportation for picking up the client for scheduled PCP appointments. 

Food Donation

Areas Of Need: Financial/Basic Needs

The client needs assistance to obtain Food donations from the community. 

Educational

Areas Of Need: Education

The client needs assistance understanding notifications and documentation from healthcare providers and government agencies. Case Managers assist the client in organizing the correspondence received. Another common educational need is how to prepare for the hurricane season, assisting the client in learning how to be ready for the hurricane season and making a plan of action.

Bank Issues

Areas Of Need: Financial/Basic Needs

The client typically needs help resolving bank issues like, missing Supplemental Security Income (SSI) deposits, obtaining bank account information, etc.
